Punctal Plugs for Dry Eye: How They Work and Who Can Benefit

If you have dry eye, your tears may evaporate too quickly, your eyes may not make enough tears, or both. Punctal plugs are a simple, in-office option that helps many patients by keeping natural tears on the eye longer. They are sometimes called tear duct plugs because they are placed in the tear drainage openings to slow tear outflow.

How Punctal Plugs Work

Tears normally drain through small openings in the inner corners of the eyelids called puncta. When plugs are placed, they partially or fully block drainage so the tear film stays in place longer. For the right patient, that can reduce burning, gritty sensation, fluctuating vision, and irritation related to dry eye.

Punctal plugs do not “create” better tears, but they can improve comfort by increasing tear availability on the ocular surface. They are often used alongside other dry eye treatments, especially when lubrication drops alone are not enough.

Types of Punctal Plugs

Your eye doctor may recommend a temporary or longer-lasting option depending on symptoms and how your eyes respond.

  • Temporary dissolvable plugs - often made of collagen, used to test whether tear retention improves comfort
  • Long-term plugs - typically silicone or similar materials, designed to be removable if needed
  • Partial-occlusion designs - meant to reduce drainage while still allowing some tear flow

What the Procedure Is Like

Punctal plug placement is quick and typically performed during a routine dry eye visit. The doctor measures the punctum to choose the correct size, then places the plug at the lid margin. Most patients feel little to no discomfort, and you can usually return to normal activities right away.

You may notice improvement quickly, or it may take a short period for the tear film to stabilize, especially if inflammation is also being treated.

Who Can Benefit Most

Punctal plugs can be helpful for a wide range of dry eye patients, but they are especially useful when symptoms are tied to low tear volume or when the eyes need added tear retention to support healing.

They may be recommended if you:

  • rely on artificial tears multiple times a day
  • have dry eye that worsens with screens, air conditioning, or seasonal changes
  • experience contact lens discomfort related to dryness
  • have dry eye symptoms after certain medications or medical conditions

Who May Need a Different Approach

Punctal plugs are not the best fit for every case. If there is significant eyelid inflammation or untreated meibomian gland dysfunction, your doctor may focus on improving tear quality first. Some patients also do better with a staged plan, such as treating the eyelids and ocular surface, then adding punctal plugs once inflammation is under control.

Possible side effects include irritation, the plug feeling noticeable, excessive tearing, or plug displacement. In most cases, issues are manageable, and plugs can be removed if they are not the right match.
